The Lead Capture form is the most critical part of your quiz funnel. It is where the "engagement" turns into a "lead." LeadQuizzes allows you to customize exactly what information you collect and exactly when you ask for it.
Adding the Form
In the quiz builder, simply navigate to the left-hand menu, find the Form Field element, and drag it into your quiz content area.
Strategic Placement
At the Start: Useful for gated content where you want to qualify users immediately.
Before the Results: (Recommended) This is the highest-converting placement. Because users have already invested time in answering your questions, they are significantly more likely to provide their email to see their personalized result.
Customizing Your Data Collection
You aren't limited to just names and emails. You can add:
Standard fields: First/Last Name, Email.
Contact fields: Phone Number.
Professional fields: Company Name.
Custom fields: Use "Short Answer" for specific data points unique to your business.
Note: Every extra field you add to a form can lower your conversion rate. Only ask for the information your sales team actually needs to follow up.
