Form Fields

Want to collect data from your content and associate it with a person or email? Then Form Fields is where you want to be.

Form fields are a LeadQuizzes feature that allows you to create a lead capture, registration, contact and any type of form you can think of. You can break the form fields up throughout your quiz or ask them all in one block. This article covers the steps for using Form Fields to create a lead form.

1. To use the form fields content element block simply click on Form Fields under Content Elements on the left side and drag it to the Content Elements box on the right side. 

You then have the ability to add the different Form Fields that you would like. To add a form field to your LeadQuizzes just click on the form and drag it to the appropriate location in the Form Builder. 

You can also edit the Button Label and enable a Skip Form Option on this screen.  

2. The Form Header element allows you to write a title and description for your form fields. You have the option to add a Title and Description text to your form header as well as the option to add an Image or a Video. 

Note: It is not Required to add a Form Header to your form. You will still be allowed to save your form without a header. However, you are required to add at least a title if you are using the Form Header element.

3. The First Name and Last Name form fields, used to capture your leads' names. You can add multiple First Name and Last Name fields to your lead form. You also have the option to make this field required.

4. The Email Form Field, which is used to capture your leads' email addresses. You can add multiple email address fields to your lead form, but you will have to specify one as the primary (lead) email address that will act as the lead contact, while the other emails will be secondary emails. You also have the option to make this field required.

Note: You won't be allowed to delete your primary email address field until you make one of the secondary email address fields the primary field. 

5. The Phone Number form field, which is used to capture your leads' phone number details. You can select a specific country or countries code that you wish to accept phone numbers from in your form field, or you can allow your users to select their country code from the entire drop-down list.  

You also have the option to make this field required.

6. The Short Answer form field, which is used to capture any form of data from your leads. You have the option to set the field entry type to either ''Text'' or ''Numeric''. You also have the option to make this field required.

7.  The Long Answer form field, which is used to capture any form of data from your leads. You have the option to make this field required.

8. The Checkbox form field, which is used to add checkbox fields to your form. You are allowed to add multiple check-boxes as well as delete check-boxes from your field. You have the option to allow multiple choice selection on this form field. You also have the option to make this field required.

9. The Drop-down form field, which is used to add a drop-down option to your form field. You are allowed to add multiple options as well as delete options from your drop-down field. You have the option to make this field required.

10. The Time form field, which is used to add a time field to your form. You have the option to set the time format by clicking on the drop-down. Time formats can be ''12'' or ''24'' hour clock count. You also have the option to make this field required.

11. The Date form field, which is used to add a date field to your form. You have the option to set the date format by clicking on the drop-down. Date formats can be in ''mm.dd.yyyy'' or ''dd.mm.yyyy''. You also have the option to make this field required.

12. The Country form field, which is used to add a country field to your form. By clicking on the drop-down in the field, you will see a list of all the countries to choose from. You also have the option to make this field required.

13. The Website form field, which is used to add a website field to your form. You have the option to make this field required.

14. The Number form field, which is used to add a number field to your form. This field can change to collect values like currency, integers, percentages, and decimals.  You have the option to set the field entry type to either ''Decimal'' or ''Percentage''. If you Select decimals then you can enter currencies, integers, and decimal numbers like 10, 12.34, $10 while for percentage the percentage sign (%) will be added automatically after your number.

You also have the option to make this field required.

15. The Hidden Field form field, which is used to keep information that is relevant to the content creator. For example (UTM parameters, Campaign name, etc.). This field will not be visible to the content taker but it will reflect in the input field in the actual content. The hidden field needs to have some content in order to be able to save.

16. Finally, you can preview your form in the design section of the builder.
